Medicare Part D helps cover the cost of prescription medications. These plans are offered by private insurance companies approved by Medicare and can be added to Original Medicare or most Medicare Supplement (Medigap) plans.
What Part D Covers
What Part D Includes
- Retail prescription medications
- Many generic and brand-name drugs
- Insulin (with capped costs under many plans)
- Vaccines recommended by the CDC
What Part D Does Not Cover
- Over-the-counter medications (unless included as an extra benefit)
- Drugs not listed on the plan’s formulary
- Drugs used for cosmetic or weight purposes
- Most vitamins or supplements
Key Components of a Part D Plan
Formulary (List of Covered Drugs)
Each plan has its own list of approved medications.
Tier Structure
Medications are placed into cost tiers (generic, preferred brand, specialty, etc.).
Pharmacy Networks
Costs may be lower at preferred pharmacies.
Coverage Stages
Part D includes four phases each year:
1. Deductible stage
2. Initial coverage
3. Coverage gap (“donut hole”)
4. Catastrophic coverage
Cost Of Part D
Typical Costs May Include:
- Monthly plan premium
- Annual deductible (if applicable)
- Copayments and coinsurance
- Drug costs based on tier
- Costs that change when you move between coverage stages
Note:
Costs vary by plan, pharmacy, region, and drug list.
How Part D Works With Other Medicare Options
Original Medicare + Part D
Helps cover prescription drugs not included in Parts A or B.
Medigap + Part D
Medigap plans do not include drug coverage, so Part D is added separately.
Medicare Advantage (MA-PD)
Most Medicare Advantage plans already include drug coverage, so a standalone Part D plan is not needed.
Enrollment Periods
Initial Enrollment Period (IEP)
When you first become eligible for Medicare.
Annual Enrollment Period (AEP)
October 15 – December 7 each year.
Special Enrollment Periods (SEPs)
For qualifying life events, such as moving or losing drug coverage.
